1. This is an average range of values and limitations of major dental PPO insurance plans. Each individual insurance plan may have some of the illustrated parameters, depending on the terms of negotiated contract.
2. Most insurances have a waiting period after the patient's enrollment in the plan, before paying for dental procedures. It can vary from 6 to 12 months for procedures under "basic" classifications, and from 12 to 24 months under "major" ones (crown, etc.).
3. Most insurances do not pay for certain dental problems, which occurred prior to the patient's enrollment in a plan. Example: The "missing tooth" clause. If the tooth was lost (extracted) prior to the time of enrollment, the dental bridge is not covered.
4. Most insurance plans do not cover certain procedures, such as dental implants, porcelain inlays and onlays, diagnostic CT scans, etc.
5. Most insurance plans "downgrade" payments for certain procedures and reimburse at much lower rate under the "alternative benefits". Example: For a porcelain, tooth-color inlay insurance reimburses only as for an amalgam (silver) filling. Example: If the patient needs a dental bridge, but has a missing tooth on the other side of the arch, the insurance pays as for a partial denture.
6. Some insurance plans impose patient's age limitations on certain procedures and restorations on permanent teeth. Example: To be eligible for payment for a dental onlay or a crown on a decayed tooth, the patient must be at least 13 years old. Example: Some insurances may exclude payment for dental bridges for patients with a missing tooth who are under age 16.
7. Most insurance plans have replacement clause exclusions on restorations and dental appliances. Example: Replacement of dental crowns, bridges and dentures is covered once every 5-10 years, with the trend moving towards 10 years.

It is an affordable way to receive the best dental care. SCDM Discount Plan is not an insurance. It is a reduced fee plan. There are no restrictions and limitations of a traditional dental insurance. All services are available immediately with significant savings. The annual participation fee is $120.00 per person. For children, age 4 to 18 - $75.00
You and your family can receive services at the same day of signing-up on the plan. All professionals at SCDM are certified, licensed and highly trained.
Provided services include:

• All dental implants are initially planned and then placed (inserted in the jawbone) under 3 dimensional (3D) CT guidance, with an absolute precision.
• Permanent porcelain restorations (crowns, onlays and inlays) are manufactured in the office while you wait and placed during the same visit. No temporaries, no impressions, no 2-3 weeks of waiting.
• All oral evaluations (dental exams) involve digital x-rays, intraoral camera images, oral cancer screening and periodontal (gums) assessment.
• Patients with periodontal disease are treated under individualized protocols, applying laser therapy.
• For patients with dental anxiety, SCDM offers IV (intravenous) sedation or general anesthesia. No anxiety, no unpleasant associations. Patients fall comfortably asleep and upon completion of treatment awake without any stress.

For businesses, there is a discounted enrollment fee, depending on the number of enrolled employees. Each and every employee shall receive the same services and discounts on treatments as privately enrolled individuals. For business participation in the plan, call 480-948-4445.
